You and up to 4 other people will play as both monsters and humans, and explore a parallel world to Toby Fox's hit games. Expect a similar style of fantastical "Echo Worlds" [as opposed to "Dark Worlds"] divided up into themed Chapters, with a similar rich lore, fun characters and primarily narrative-based TTRPG experience as opposed to DnD 5e's combat heavy experience. However, expect also the unexpected. The same way, on release, that Deltarune shocked you as an Undertale fan - expect Alterhearth to have the same twists and turns, as well as a dark mystery at its heart...
